Overview of Facility
The Christian Health Care Center (CHCC), a non-profit organization located in Wyckoff, New Jersey, consists of five elder-care/senior residences: Heritage Manor (SNF), Southgate (skilled behavioral), Longview Assisted Living Residence (ALF), Hillcrest Residence (boarding house), Evergreen Court (senior apartments) and two Adult Day Services programs (Wyckoff and Wayne). CCHC went live in October 2008 with SigmaCare’s full electronic medical records (EMR) system including computerized physician order entry (CPOE), medication/treatment administration records (eMAR/eTAR), progress notes, 24-hour report, minimum data set (MDS), care planning, user-defined assessments and CNA assignments.

In order to improve quality of care, reimbursement and operational efficiencies, CHCC prepared their staff for the transition from paper to automation and encouraged 100% participation during SigmaCare’s complete implementation process resulting in optimal user adoption and compliance rates. Additionally, CHCC utilized SigmaCare’s dashboard and reporting features to monitor operations and quality measures. For example, CHCC monitors documentation rates and resident-specific problem areas and makes adjustments as needed. CHCC also benefits from SigmaCare’s interoperability through partnerships with pharmacies and billing vendors.
With a combination of effective operational management and active quality monitoring, CHCC improved compliance, lowered operational costs and increased reimbursement, for example:
